Preparing For Your Appointment

We are making a few changes to streamline your appointment experience and implementing some virtual measures to help protect our patients and our staff. Please read below our three step process for how to prepare for your upcoming virtual or in person appointment.

Email Communication Policy

Please read our email policy prior to participating in any email communication with our office. By commencing email communication with our staff you are agreeing that you have read, agree, and understand the policy.

Step One - Medical History

If you are a new patient OR any of your medical history or medications have changed, please fill out the "New Patient Questionnaire" form and email it to us at appointments@drcalotti.com


Step Two - Visual Acuity Check

Prior to your appointment, we will ask you to check your vision at home. Please choose one of the following options to complete this task. Please include the results of your vision test in your email.

 

If you have access to a computer and a printer, please download the following vision chart and follow the included instructions to check your vision

  • http://cdna.allaboutvision.com/pdfs/snellen-chart.pdf
  • Print out the chart
  • Place it on a wall or stand 10 feet away from you
  • Cover your left eye and read the lines with your right eye. Note down the smallest line you were able to read with your right eye
  • Cover your right eye and read the lines with your left eye. Note down the smallest line you were able to read with your left eye
 

If you have an iPhone, please download the following app to check your vision on your cell phone

  • Eye Chart Vision Screening (by Dok LLC) available on the App Store
  • The app should open to a vision chart with rows of letters (Snellen chart)
  • Place/hold the phone 4 feet away
  • Cover your left eye and read the lines with your right eye. Note down the smallest line you are able to read using the numbers to the left of each line (20/40, 20/30 etc)
  • Cover your right eye and read the lines with your left eye. Note down the smallest line you are able to read using the numbers to the left of each line (20/40, 20/30 etc)
 

If you have an Android smartphone, please download the following app to check your vision on your cell phone

  • Eye Test (by Designveloper) available on the Google Play Store
  • Open the app and click on the "Visual Acuity" tab
  • Click on the button to test your right eye (cover your left eye for this)
  • Record the number the app gives you for right eye vision (20/20, 20/30 etc)
  • Click on the button to test your left eye (cover your right eye for this)
  • Record the number the app gives you for left eye vision (20/20, 20/30 etc)
 

Step Three - How to take a good photo of your eye

Prior to your appointment please follow the below instructions to send us a photo of each eye. Please send photos to appointments@drcalotti.com

 

If you have a family member or friend to help:

  • Use the rear camera on your smartphone
  • Turn the flash ON
  • Have one eye centered in the screen
  • Use 2.5X zoom - your eye should fill most of the screen
  • Hold the phone about 3-4 inches from the eye
  • Take photo and repeat for the other eye
  • **ensure the eye is in clear focus ?? if the photo is blurry, clean the camera lens and retake the photo

If you are alone:

  • Use a mirror in the washroom or other well lit room
  • Use the rear camera (face the phone so you can see the phone screen in the mirror)
  • Turn the flash ON
  • Use 2.5X zoom
  • Using the mirror, ensure one eye is centered in the photo and that your eye fills most of the screen
  • Take photo and repeat for other eye
  • **ensure the eye is in clear focus ?? if the photo is blurry, clean the camera lens and retake the photo

We understand that unforeseen events sometimes require missing an appointment, however without notifying us 48-hours in advance, you will be subject to being charged an additional fee to reschedule.

If you are a healthcare professional

New patient referral form, or Urgent Retina Referral Form and fax to our central intake fax at 519-752-3277.

We always try to accommodate all referrals promptly, and try our best to shorten waiting time. To utilize every appointment slot, we therefore request 48-hours notice to cancel appointments. We understand that unforeseen events sometimes require missing an appointment, however after missing your second appointment without notifying us 48-hours in advance, your patients will be subject to being charged an additional fee to reschedule.
