New Patient Information

At the Great Lakes Eye Centre we want to be sure to provide you with the best information and guidance to better your healthcare as our patient. Here are a few tips to prepare you for your visit:

Please bring to your appointment with Dr. Calotti:

  • A valid OHIP card
  • A list of your current medications and any allergies you may have. You will also need to know the name of your family doctor if you have one
  • Current glasses, both distance and reading
  • You will need someone to drive you home from your appointment as your eyes will be dilated on your first visit
We understand that unforeseen events sometimes require missing an appointment, however after missing your second appointment without notifying us 48-hours in advance, you will be subject to being charged an additional fee to reschedual.
