Masks are required to be worn for all office visits until further notice. Patients will need to bring their own mask to any appointments. If you do not already have a mask, you can click here for instruction to make a homemade mask. Masks are also available for purchase at these locations:

  • Rexall St. Joe's Life Care Building
  • Rexall Paris
  • Rexall St. Paul Avenue
  • Rolston's Delhi

A valid OHIP card.

Current glasses (distance and reading).

You will need someone to drive you home from your appointment as your eyes will be dilated on your first visit.

You will need a family member/companion to accompany you for any language barrier, medical conditions (such as dementia), surgical appointments including days of surgical consultations, surgical planning or surgery day.

Please be advised that there may be some recommended testing that are not OHIP covered.

Parking Information

Convenient outdoor, gated parking is available beside the main St. Joe’s building, accessible from Chatham Street.

Rate: $5 (Cash only), (System: Token-operated).

Exit Tokens: Please pick up a token at the St. Joe’s front desk before leaving the lot.

Regarding Uninsured Service Fees

Please note that the Ontario Health Insurance Plan (OHIP) covers medically necessary physician and hospital services. However, certain administrative tasks and non-medical services are not funded by OHIP.
The following fees listed are examples of such uninsured services:
Block Fee – $75: This fee often bundles several potential uninsured services (like prescription renewals by phone/fax, some forms, etc.) for patient convenience over a set period, usually a year. These individual services are not covered by OHIP.
Transfer Records – $50: This charge covers the time and resources required to copy and securely transfer your medical records to another provider, yourself, or a third party upon your request. This administrative task is not an OHIP-insured service.
Payment for these uninsured services is the responsibility of the patient.

Missed Appointment Fees

We always try to accommodate all referrals promptly, and try our best to shorten waiting time. To utilize every appointment slot, we therefore request 48-hours notice to cancel appointments. We understand that unforeseen events sometimes require missing an appointment, however after missing your appointment without notifying us 48-hours in advance, you may be subject to being charged an additional no show fee of $150
